How to Sign Up for Livara Health

Follow these steps to create your Livara Health account and get started with your program.

Important: If you were referred to Livara through One Medical please visit this article: One Medical Account Activation

You can download the Livara Health App on your smartphone, or sign up on the website.

App: You can download the application on your Iphone/Android via the Appstore/Google play store

Select Sign Up
Enter your First and Last Name exactly as they appear with your insurance.
Enter your Date of Birth and Member ID
- If your member ID has any letters in the first 3 characters please exclude the first 3 characters
  - For example (9A2123456789) you would type in (9A2123456789) instead
Finish the rest of the registration

Read and accept Livara Health’s Terms of Service and Privacy Policy to continue

You can now access the Livara platform without completing your intake form
If you skip it for now, you’ll see reminders to complete it later for a full program experience

Go to member.livarahealth.com or open the Livara Health app
Use your email and password to log in
If you have any trouble, reset your password
