Re-Enrolling in Livara

If you were previously enrolled in Livara and want to rejoin the program, here’s what to do:


1️⃣ Check Your Situation

  • Email support@livarahealth.com to request re-enrollment

    •  We require which body part you would like to re-enroll for

  • We’ll confirm eligibility with your insurance before proceeding (This can take 10-20 business days)

  • We’ll manually archive your old account before you create a new one

2️⃣ Things to Know

  • You cannot re-use a discharged account

  • All intake forms and appointments will need to be completed again


Need Help?

Email support@livarahealth.com or submit a ticket and include:

  • Your full name and DOB

  • Insurance provider and Member ID

  • If possible, the approximate date you were last enrolled
