If you were previously enrolled in Livara and want to rejoin the program, here’s what to do:
1️⃣ Check Your Situation
- Email support@livarahealth.com to request re-enrollment - We require which body part you would like to re-enroll for 
 
- We’ll confirm eligibility with your insurance before proceeding (This can take 10-20 business days) 
- We’ll manually archive your old account before you create a new one 
- Then you must register with a new account. Instructions on how to do so can be found in this article. How to Sign Up For Livara Health 
2️⃣ Things to Know
- You cannot re-use a discharged account 
- All intake forms and appointments will need to be completed again

Email support@livarahealth.com or submit a ticket and include:
- Your full name and DOB 
- Insurance provider and Member ID 
- If possible, the approximate date you were last enrolled
